* Provide general treatment room services including wound management, dressings, cervical screening, and immunisations (adult, childhood and travel)
* Deliver chronic disease management reviews (e.g., diabetes, asthma, hypertension, COPD, CHD)
* Implement and evaluate evidence-based treatment plans
* Identify and manage patients at risk of long-term conditions
* Assist GPs with minor procedures when required
* Maintain accurate clinical records using System One
* Provide opportunistic health promotion and lifestyle support
* Contribute to Infection Prevention & Control processes and audits
* Work within PGDs and practice policies
* Participate in shared learning, audit activity, and continuous improvement

Ashbourne Medical Practice is a large teaching and dispensing practice serving a growing population of 10800 patients from a modern, purpose-built premises in the centre of Ashbourne. Our multi-disciplinary team includes GPs, ACP/ANPs, a practice nursing team and supportive non-clinical teams. Experience
* Experience of working in accordance with the NMC Code of Professional Conduct (2002) and The Scope of Professional Practice (1992).
* 12 months experience of working in Primary Care.
* Experience managing long-term conditions.
* Experience of working autonomously.
* Experience of a clinical system e.g. TPP SystmOne or EMIS.
* Experience of Microsoft Office software.
* Knowledge of issues affecting General Practice.
* Experience of QOF and chronic disease management templates.
* Experience using System One.
* Knowledge of health promotion strategies.
