Job Summary

We have an exciting opportunity to join our multi-disciplinary PCN Team due to changes in the ARRS Scheme. We are seeking a newly qualified General Practitioner to work alongside our practices and PCN staff. Our team includes Clinical Pharmacists, Social Prescribers, Health & Wellbeing Coaches, Mental Health Practitioners, Paramedics, First Contact Physiotherapists, GPAs, Pharmacist Technicians, PCN Manager/Digital & Transformation Lead, and a Care Home Team.
The successful candidate will support healthcare professionals, patients, and carers within the PCN and practice settings.
Location
Based at one of the following sites: Roslea Surgery, Riverside Medical Centre, The Ryan Medical Centre, New Longton Surgery, Lostock Hall Medical Centre, or other associated PCN sites including Care Homes and Home Visits.

Main Duties
* Provide autonomous clinical care, including assessments, diagnoses, and treatments in primary care.
* Support patient education, develop care plans, and collaborate with the healthcare team.
* Maintain accurate documentation and participate in audits.
* Prescribe medications following formulary guidelines.
* Visit patients at home or care homes as needed, including emergency visits.
Patient Management
Manage patient care efficiently, balancing continuity and access, utilizing technology, and communicating effectively to manage expectations and safety concerns.
Confidentiality & Data Protection
Respect patient confidentiality, handle sensitive information responsibly, and comply with relevant data protection legislation.
Person Specification
Qualifications (Essential)
* Qualified GP with MRCGP, CCT, and full GMC registration.
* Experience in primary care, with competencies in clinical audit, QOF, and risk management.
* Full UK driving license and own vehicle for home visits.
Desirable qualifications include medicines management experience and familiarity with ICB initiatives.
Additional Requirements
This role is subject to a DBS check and requires current UK professional registration.
