We are looking for individuals with exceptional caring and interpersonal skills with an empathic and compassionate nature to join our team. We work predominantly across the Derbyshire, Derby City and Nottinghamshire area.
As a Health Care Assistant in our team, you will deliver care and support to people with terminal illnesses in the last few months and weeks of life. We carry out a range of care duties including helping patients meet their hygiene needs, monitoring symptoms and keeping detailed records and emotional support to the patient and their families.
Our approach is predominantly one to one, so you will often be working independently using your own initiative in line with patient need and Registered Nurse support.
What we are looking for individuals with:
Relevant palliative care experience
S/NVQ Level 2 or above.
UK driving licence & access to a vehicle are essential.
Main duties of the job
TreetopsHospice provides a nursing service, which is available 7 nights a week, forpeople with a limited life expectancy, to enable them to remain at home if thatis their wish. It is intended to be complementary to existing communityservices.
Wework in co-operation with the Primary Health Care Team and other care agenciesto provide support and skilled nursing care to those in the terminal phase oftheir illness, or with chronic incurable conditions.
Our key role within the department is to provideindividualised palliative care to patients as part of the Hospice at Home nightservice, either through the provision of one-to-one night support in apatients home or as part of a roaming team of two covering the SouthernDerbyshire geographical area.
To providerespite, help and support to the families and carers of individual patients.
To attendteam meetings on a regular basis, undertaking clinical supervision, mandatoryand on-going training and education sessions as required.
Toparticipate in team peer review and support through reflective practice.
