Job Description:
Liquid Personnel is currently recruiting an Adult Social Worker on behalf of its client based in Warrington.
What will your responsibilities be?
Adopting a Discharge to Assess approach.
You will facilitate safe and timely transfer from hospital to home or the community and where an interim community placement is required.
You will follow the service user, building on your initial assessment to plan for service users' longer-term needs.Benefits:

To be successful in this role you must have,
Social Work England registration
Eligible to work in the UK
Hold a full UK driving licence
Post-qualified experience
