Working as a Health Care Assistant for Leeds Community HealthcareTrust is different from anything you will have done before; we work as acommunity within a community. The Wharfedale Recovery Hub, Bilberry and Heatheris located in Wharfedale hospital 2nd floor, Newall Car Road LS21 2LY Otley andis for patients who require a short stay for up to 6 weeks after beingdischarged from hospital. We provide rehabilitation and plan for their safedischarge to either or to their own home or to a nursing/care home. Our aim isto work with our patients and help them achieve their recovery goals with thesupport of our nursing and therapy team, which includes OccupationalTherapists, Speech and language Therapists, Physiotherapists, Dieticians,Registered nurses, GP and Health Care support workers.
The hub operates 24 hours a day, 365 days a year and you willsupport the patients on various shift patterns to maintain this operation.
Main duties of the job
The post holder will beproviding direct and indirect patient care independently but with supportavailable from a supervisor within the clinical area. In doing so, they willsupport the assessment of patients and implementation of programmes of care delegatedto them by the appropriate staff member ensuring that they practice withintheir sphere of competence and knowledge. The post holder will work within arange of procedures and pathways in which they have received training to becompetent.
Assisting with dailyliving activities such as eating, showering and using the toilet
Utilising equipment tomove patients when necessary
Helping tidies thepatients room
Taking the patients vitalsigns such as blood pressure and temperature
Talking to patients andreassuring them
Attending meetings withother healthcare professionals
Assisting nurses and otherstaff when needed
