The successful candidate will be based within the Integrated Discharge Hub - Hospital Social Work Team delivering a service to patients in St John’s Hospital to help facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need sort term support due to a change of circumstances
The Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care co-ordinators, reablement staff, discharge facilitators, AHP staff as well as representatives from different services.
This is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.
Tasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to undertake essential training including in adult support and protection training.
The worker must have the ability to travel throughout West Lothian.
SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential.
