Responsibilities
* Act as an expert nurse/practitioner for patients and families, providing advice and consultancy to empower other professionals to deliver high quality palliative/end‑of‑life care.
* Work autonomously to provide holistic assessment and management for patients with complex problems and their families, enhancing quality of life and enabling choices about care.
* Monitor, evaluate and review palliative interventions.
* Act as a patient advocate and support informed discussions around treatment options affecting quality of life, with preferences around place of care and place of death.
* Work collaboratively as part of a multi‑disciplinary team, supporting colleagues and respecting their contribution to meetings.
* Possess organisational knowledge of Trust protocols and procedures and adhere to them.
* Be responsible for providing accurate records of information required by the Trust.
* Contribute to quality initiatives and service development.
* Maintain sound clinical knowledge of all aspects of palliative care and palliative medicine, including benefits and reactions to treatments and drugs used for symptomatic and therapeutic benefit; provide expertise in symptom control and offer appropriate advice.
Person Specification
Essential
* Good standard of literacy and numeracy.
* Advanced communication skills.
* Ready to undertake training or commitment.
* Significant experience (2 years or longer) working at a senior level (band 6 and above) in hospital or community settings.
* Experience of teaching and mentoring/coaching staff.
Desirable
* Teaching qualification.
* Any additional relevant experience.
Benefits
* High Cost Area Supplement – Outer London: 15% of base salary (minimum £4,870, maximum £6,137 per annum, pro‑rata for part‑time).
* Subject to a probationary period.
