Job Description
The Local Area Coordinator will support Adult Social Care teams by identifying resources, groups, services, and organizations within the Cheshire East area that will benefit them. The service is available to all Adults with Care Act eligible needs following assessment. The Coordinator will coordinate information on available community support within the Cheshire East Council (CEC) area and will inform community teams and the LiveWell site.
A key role is to enable and support Adults with Care Act eligible needs, as well as those on the margins of eligibility, to support the wellbeing and prevention areas of The Care Act. The Coordinator will facilitate access to existing community support rather than relying solely on funded CEC services.
They will also undertake assessments when required and identify low-level equipment, assisted technology, and access to Reablement services.

The role involves working closely with community colleagues in multidisciplinary teams to ensure access to local resources for those with Care Act eligible needs. Experience working with adults in the community, ability to work independently, and knowledge of the local area are desirable. The role covers anywhere within Cheshire East.
We seek enthusiastic, creative individuals who can quickly identify needs and facilitate support. The role involves working with Social Workers, Social Care Assessors, and Occupational Therapists, with a preference for a four-day workweek. A hybrid working model is in place, combining office-based and remote work, with visits within the local area. An enhanced DBS check is required.
