*About You*
We are looking for a graduate Occupational Therapist who is looking for experience of working across Health and Social Care on our mixed rotation. This is an exciting opportunity to work as an Occupational Therapist across the four health and social care organisations in Leeds.

*About The Role*
Occupational Therapy in Leeds is leading the way by working across boundaries, and we are proud to have an innovative mixed rotation program. We are currently recruiting for an Occupational Therapist to join the rotation, which spans across the four health and social care organisations: Leeds and York Partnership Foundation Trust, Leeds Teaching Hospitals NHS Trust, Leeds City Council, and Leeds Community Healthcare. You will have the opportunity to work within all of these partner organisations. Each rotation lasts 9 months.

*We Are Looking For Individuals Who Are:*

* Passionate about occupation and its role in mental health and physical health
* Flexible, including being able and willing to work shifts and weekends depending on the placement
* Able to work in various teams and on different sites across Leeds

The mixed rotation offers a great career start in Occupational Therapy, with opportunities to work with various groups of service users, in both physical and mental health settings, and across community and inpatient environments. Each rotation is for nine months.

You will be supported both personally and professionally through clinical supervision, training, and other Continuing Professional Development (CPD) opportunities. Each partner organization offers established programs for new graduates, providing specific support and development throughout your first year.

Leeds Adults and Health provides an assessed year of supported employment (similar to Preceptorship) to support your transition into employment.

Successful applicants will be employed through their recruiting organization and will rotate between partner organisations.

*Job Purpose*
Under the general direction of a Team Manager, and with support from Senior Occupational Therapists, this role involves providing a high quality and comprehensive Occupational Therapy service to adults, children and young people, carers, and the community in accordance with organizational needs, policy and practices of the Directorate, and statutory guidelines. The role provides different learning experiences for Occupational Therapists on rotation.

Responsibilities must always be carried out within the City Council's Policies and Procedures, particularly those relating to Equality and Diversity, Health and Safety, and within the Health and Care Professions Council Code of Practice.

*Responsibilities*

* Undertake Occupational Therapy assessments suitable for someone with post-qualification experience, focusing on occupational aims to reduce barriers to inclusion.
* Conduct re-enablement assessments with an occupational emphasis and be responsible for care planning throughout the process.
* Organize work activities, prioritizing tasks and responsibilities to ensure statutory legislation and service priorities are met.
* Manage an allocated caseload of disabled people with supervision; regularly monitor, review, and evaluate changes in their needs to ensure that all services contribute effectively to support plans, making efficient use of resources.
* Complete moving and handling and hoist training, and, with appropriate guidance from senior staff, carry out proper moving and handling assessments.
* Contribute, under clear supervision, to the enhanced screening of new referrals and people already receiving services.
* Based on assessed needs and risks, provide or recommend services to disabled people that remove barriers to social inclusion, using a person-centred approach.
* Develop, implement, and review outcome-focused Occupational Therapy interventions to meet assessed needs and promote independent wellbeing and inclusion.
* Identify the specialist housing needs of disabled people and their families and support them in accessing suitable housing or adaptations.
* Deliver, fit, and collect equipment safely, including handling equipment as necessary.
* Provide concise, informative verbal and written reports based on a comprehensive analysis of complex information.
* Work according to the prescribed standards of the College of Occupational Therapists.
* Maintain accurate and up-to-date case notes and other records; write reports as required.
* Utilize management information systems effectively, update them appropriately, and provide concise and accurate information about circumstances and plans within the limits of data protection.
* Participate in meetings and relevant forums appropriately.

Specific Responsibilities within the Blue Badge Eligibility Assessment Team

* Conduct in-clinic assessments for Blue Badges utilizing the Department for Transport Guidelines.
* Contribute to telescreen assessments.
* Undertake visits to establish eligibility for Organisational Badges.

Specific Responsibilities within Children and Young People Services

* Consider the development of the child or young person and the needs of the family/carer when making service provision recommendations.
* Obtain the views of the child or young person and the family/carer during the assessment process, considering these views when making service provision recommendations.

Specific Responsibilities within Environment and Housing

* Understand and contribute to the annual service plans of the Health and Housing service that facilitate opportunities for disabled individuals and their families to live independently in quality, affordable, safe, and stable housing across all tenures.
* Possess knowledge of the various cross-tenure funding streams and the scope of the adaptations and re-housing services within Council.

Specific Responsibilities with the Occupational Therapy Recovery Team

* Work on a rota pattern between 8 am and 8 pm, 7 days a week, including some weekend working.
* Provide Occupational Therapy advice and support to the SkILS Reablement Team outside office hours (5 pm-8 pm Monday to Friday and 8 am-8 pm at weekends).
* Lead in the discharge planning for customers in the Recovery Hubs.

Specific Responsibilities for Integrated Reviewing Team

* Be part of a citywide integrated reviewing team and conduct reviews for customers who meet the agreed criteria.
* Undertake moving and handling risk assessments with customers and carers (both paid and unpaid) and make recommendations to reduce care packages through process or equipment changes.
* Update support plans on CIS and liaise with CCC as necessary.
* Provide detailed moving and handling plans to care providers along with advice, support, and guidance.
* Work with care agencies to ensure staff are competent in carrying out single-handed care/optimized handling.
* Collaborate with social workers in the Integrated Reviewing Team.
* Adhere to agreed KPIs for numbers of assessments to be completed weekly.
* Provide detailed performance information weekly, evidencing savings made and any expenditures on equipment.

Specific Responsibilities for Early Intervention Team

* Provide enhanced screening for new contacts to the council who meet the criteria for the Early Intervention Team.
* Promote Independence and Wellbeing by intervening early to support individuals, helping them retain their skills and confidence, and preventing the need for or delaying deterioration wherever possible.
* Embed reablement principles within Early Intervention Teams; establish a strengths-based approach, focusing on promoting independence.
* Make appropriate referrals to the area Disability Services Teams.
* Provide detailed performance information to evidence savings made.

*Qualifications*
Appropriate professional qualification Degree/Diploma in Occupational Therapy. Registration with the Health & Care Professions Council (HCPC) upon post commencement and maintain yearly registration.

Knowledge

* Qualified as an Occupational Therapist.
* Registered with the Health and Care Professions Council (HCPC).

* Of relevant legislation addressing disabled people's needs from diverse backgrounds.
* Aware of factors impacting disabled individuals and families, such as physical and mental health conditions.
* Understanding statutory and voluntary agencies' roles and responsibilities.
