About the Service
An exciting opportunity has arisen to join Devon County Council’s Bridges Service (formerly Edge of Care).
The Service brings together a range of disciplines to work intensively and flexibly (including evenings and weekends) to provide timely and creative support to meet the complex needs of young people and their families and to prevent family breakdown. The team works intensively and holistically with families, primarily in the home, to support change.
About the Role
As an Adolescent Worker You will provide intensive support to young people and families with multiple and complex needs and subject to a range of other statutory and voluntary interventions. Supporting young people with appropriate skilled interventions and practical support, both in response to crisis and for some families, as part of a longer-term support package.
You will be working as part of a locality team, working in partnership with other professionals to deliver timely support to young people and families. This includes developing and coordinating planned interventions using the Bridges Support Agreement and review process. Utilising the Bridges practice model, you will have the responsibility for coordinating interventions, including the analysis and formulation of plans of support involving provision of data & reports, and supporting social work plans.
About You
We are looking for experienced applicants, from a range of backgrounds, who are passionate about achieving the very best outcomes for adolescents and their families. You will need the skills to engage and build meaningful relationships with young people based on restorative principles and provide evidence-based interventions to support sustainable change. You will need to be persistent, tenacious and assertive, working with young people who may be resistant to support from a statutory agency.
You should possess a wide range of skills and knowledge, including:
1. Excellent Communication and Engagement skills as this work will involve direct contact with young people and their families
2. Experience of multi-agency working and able to develop and maintain strong relationships with professionals from a range of agencies.
3. The ability to listen and use a restorative approach to support and challenge families to make positive changes in their lives.
4. Be enthusiastic and motivated to work creatively with young people, parents and other professionals.
