We are looking for a proactive, positive, supportive, and reliable qualified Practice Nurse to join our current Nursing team. We welcome interest from newly registered Band 5 Practice Nurses (conversion to General Practice Nursing available if necessary) or Specialist Band 6 Practice Nurses with a particular interest in Chronic Disease Management.

Main duties of the job
The role includes all aspects of clinical nursing such as wound care, cervical cytology, vaccinations, and immunizations. Specialist nurses will have additional responsibilities including long-term condition reviews, particularly in Respiratory.

Our in-house services include LARC, joint injections, occupational health, medicals, and acupuncture.
We are an established training practice hosting FY2s, GP VTS, paramedic and nurse practitioner trainees, as well as medical students, with strong links to Portsmouth Medical School.
Job responsibilities
Provide nursing services to a defined practice population, collaborating with the Practice Nursing Team and GP colleagues. This includes clinical duties, health promotion, health screening, and contributing to service development and clinical audits. Work with GPs and Specialist Practice Nurses to develop and implement the Practice Development Plan and meet quality indicators within the NHS Contracts Quality and Outcomes Framework.
KEY RESPONSIBILITIES
Clinical
1. Assess, plan, and provide healthcare and nursing services to the practice population, ensuring they are effective, relevant, evidence-based, and of high standard.
2. Provide comprehensive general practice nursing services, including venepuncture, wound care, diagnostic tests, ear care, health checks, assessments for patients over 75, travel immunizations, and advice, both within the surgery and in patient homes where appropriate.
3. Achieve and monitor breast and cervical screening targets, including recording and call/recall management.
4. Deliver health promotion services such as smoking cessation and healthy lifestyle clinics.
5. Administer children's immunizations according to schedules and record results accurately.
6. Coordinate annual influenza and pneumococcal vaccinations, targeting vulnerable patients to meet local and national targets.
7. Administer medicines in accordance with Patient Group Directions, ensuring professional responsibilities are met.
8. Assess patient understanding, gain informed consent, and work within NMC guidelines, especially for patients lacking capacity.
9. Participate in clinical governance, managing risks, attending meetings, and handling complaints.
10. Collaborate with primary care, community nurses, allied health professionals, and social services to ensure high standards of care.
11. Assist in emergency cleaning of bodily fluids following infection control policies.
