An exciting opportunity has arisen for an experienced and motivated nurse to take on a newly established role as the Advanced Nurse Practitioner for the South London Cross Trust Amyloidosis Service Project, embedded within the Trust’s wider Inherited Cardiac Conditions (ICC) service.
The objectives of the 12 month project are to improve the care of patients with cardiac amyloidosis by:
• Developing and coordinating the clinical pathways to review and optimise patients with a suspected or confirmed diagnosis of cardiac amyloidosis.
• Develop specialist clinics for rapid assessment of cardiac amyloidosis patients.
• Provide and coordinate access to specialist diagnostics.
• Accelerating initiation of appropriate licensed treatments for those patients diagnosed with all forms of cardiac amyloidosis under the guidance of an expert clinician.
This post will support to co-ordinate Amyloid clinics, aiming to ensure that there is a thorough and coordinated clinical pathway for patients with either suspected or confirmed cardiac amyloidosis (CA) to access local multidisciplinary teams and specialist
diagnostics. Based within Guys and St Thomas' NHS Trust and covering Kings Health Partners sites, this post reflects the continued development of specialist services and will play a key role in shaping and delivering high-quality care for patients with suspected and confirmed cardiac amyloidosis across a coordinated, cross-site pathway.

Main duties of the job
The post holder will practise as an advanced practitioner, working with a high degree of clinical autonomy to assess, plan, and manage care for patients with complex cardiac conditions. You will provide expert clinical leadership across the pathway, including interpretation of diagnostic investigations and coordination of care across inpatient and outpatient settings. A central component of the role is close collaboration with heart failure and haematology teams, ensuring integrated care for patients whose management spans both cardiac and systemic disease pathways. You will act as a specialist resource to the multidisciplinary team, supporting complex decision-making and delivering safe, effective, and patient-centred care.

In addition to clinical responsibilities, you will support the ongoing development of the cardiac amyloidosis pathways as part of the broader ICC model, contributing to pathway design, service improvement, and quality initiatives. You will work collaboratively with regional networks and national partners, including the National Amyloidosis Centre, to ensure alignment with best practice and equitable access to specialist services. This role also offers opportunities to contribute to education, research, and innovation, supporting the translation of evidence into practice and enhancing staff capability across the Trust. You will be joining a collaborative multidisciplinary team with strong links across cardiology, heart failure, and haematology, and will play an important role in strengthening the Trust’s position as a leader in specialist cardiac care.

Qualifications/ Education
Essential criteria
1. Bachelors Degree in nursing, health or management related studies.
2. Post registration qualification or equivalent post qualification experience in cardiac specialty
3. A Masters level qualification/in progress
4. Teaching and Assessment qualification
5. Advanced physical assessment skills and qualification
6. Ability to carry out genetic counselling and testing; holding a genomics-based postgraduate qualification (level 7)
Desirable criteria
7. Be an independent prescriber according to the Trusts Non medical prescribing policy

8. Broad clinical experience relevant to the post, including specialist skills and experience in specialty.
