Personal Advisor – Leaving Care Team

Lambeth Council is seeking a dedicated Personal Advisor to join our dynamic Leaving Care Team. This is a critical role central to delivering an effective, integrated, and holistic Aftercare Service in line with the Children (Leaving Care) Act 2000 and the Children Act 1989.
Key Responsibilities
As a Personal Advisor, you will be crucial in coordinating support across multiple domains:
* Pathway Planning: You will work in partnership with young people, their social worker, carers, and other relevant agencies to co-ordinate, monitor, and review Pathway Plans for all allocated young people as they move towards independence in Lambeth.
* Multi-Agency Partnership: You are responsible for developing close and effective working relationships with key agencies, to facilitate a co-ordinated multi-agency response, and actively support and encourage inter-agency pathway planning.
* Housing & Financial Stability: A primary focus is to support young people to sustain their accommodation. This includes providing support to prevent young people going into rent arrears. You will draw up minimisation and engagement plans to prevent repossession and work closely with the benefit agency and housing officers to assess eligibility and entitlements and ensure they are provided with appropriate service.
About You
You must possess proven experience supporting vulnerable young people, a comprehensive knowledge of the relevant legislation, and the ability to work collaboratively with a diverse range of partners. You must be an excellent communicator, highly organised, and possess a non-judgmental, restorative approach
