📍Based in Liverpool, supporting individuals across the local community.
What you'll be doing:
* Providing person-centred support to individuals with learning disabilities and/or mental health conditions
* Assisting with daily living activities, community engagement, and personal care where required
* Supporting individuals to achieve their goals, build independence, and live fulfilling lives
* Working collaboratively with healthcare professionals, families, and wider support networks
* Maintaining accurate records and contributing to care plans
* Promoting dignity, choice, and respect at all times
What we're looking for:
* Experience in a health, social care, or support work setting to join our experienced team
* A genuine passion for supporting people with LD and/or mental health needs
* Strong communication skills and a patient, empathetic approach
* Ability to work flexibly, including evenings, weekends, and bank holidays
* NVQ/QCF Level 2 or 3 in Health & Social Care (desirable, or willingness to work towards
