Role Overview
We are recruiting a band 6 nurse to work as part of our In‑Reach Team based at Parklands House, Basingstoke. The team is part of the larger Older Persons Community Mental Health Team and is specialist in working with people in care homes. The successful candidate will have a wealth of experience, skill and knowledge in mental health in older people, especially dementia.
Responsibilities
* Provide specialist assessment and support to people with complex cognitive impairment or other mental health problems.
* Maintain a safe and stable environment for the patient by supporting the care home staff with person‑centred care.
* Co‑ordinate complex care, discuss clinical assessments with the team and supervise a Community HCSW.
* Formulate care and risk‑management plans in collaboration with the service user, care home staff and where appropriate, their relative.
* Actively consider and participate in team development while working under the supervision of the Clinical Manager.
Qualifications
* Valid driving licence suitable for use in the United Kingdom and access to a vehicle for work purposes.
* Excellent communication skills, determination to advocate on the patient’s behalf and ability to manage a caseload safely.
* Sound clinical skills, ability to coordinate complex care, good administration and IT skills, report writing, organisational skills and safeguarding.
Working hours are Monday to Friday 9:00 to 17:00.
