Job summary
The post holder will be a member of the Immunisation Team, responsible for delivering immunisations as part of the Department of Health (DH) programme, with a particular emphasis on the school-age vaccination programme.
Main duties of the job
1. Assess, plan, deliver, and evaluate evidence-based, quality care for an allocated group of patients on a shift basis.
2. Work collaboratively with patients and their families, involving them in care decisions.
3. Recognize deterioration or changes in patient conditions requiring intervention and refer appropriately.
4. Maintain effective communication and relationships within the multidisciplinary team.
5. Provide early intervention for safe patient discharge, collaborating with relevant teams to prevent unnecessary readmissions.
6. Act as an advocate for patients and families, promoting privacy, equality, diversity, and inclusion.
7. Uphold professional standards and Trust values, promoting a culture of care.
8. Supervise non-registered colleagues, including healthcare support workers and student nurses.
9. Ensure completion of safety and role-specific training as per Trust policies.
10. Take responsibility for personal professional development and performance.

Job description
Plan and organize immunisation sessions in schools, community settings, and homes. Deliver the school-age immunisation programme following medicines management policies. Provide advice and support to schools, promote the DH Immunisation Programme, and support health promotion activities. Immunise school-aged, pre-school, and adult populations. Work within safeguarding policies, promote positive health, and support families to improve immunisation uptake. Coordinate healthcare for children with complex needs, target vulnerable groups, and support healthy lifestyle choices. Ensure appropriate care during all immunisation stages, including emergencies, and provide confidential support regarding immunisation and related conditions.
