Overview
Could you help to enable and empower families of children with special needs and disabilities? If you have experience working with the whole family, then we have an ideal job. This opportunity is a chance to positively change the lives of families and children and to gain valuable experience and help develop your career. The children or young person will have a severe and enduring learning disability and may include physical disability. This is not a hands on health role providing personal care for the young person but is about enabling the family. You will complete an assessment with each family on your caseload and develop an Early Help plan that identifies the support needed. This may include helping access financial support, mental health support or support in the community.
Responsibilities
• Complete assessments with families and develop Early Help plans that identify and coordinate the support needed.
• Enable the family to access the right services and support (financial, mental health, community) as part of an holistic approach.
• Work collaboratively with a committed management team to deliver high-quality family-focused support.
• The role focuses on empowering families rather than direct personal care for the child or young person.
Qualifications and experience
* Experience of working with children and the whole family.
* Knowledge of safeguarding children and young people.
* Experience of assessing needs of children, young people and families.
