Job summary

This is a new innovative 10 PA split post between South Durham (North and South Dales) Community Adult Mental Health Treatment and Intervention Team and the ADHD clinic. The post provides excellent opportunity for experience in both community and ADHD team.

South Durham (North and South Dales) Community Adult Mental Health Treatment and Intervention team is based at The Goodall Centre, Bishop Auckland. The Team provides secondary mental health service input for all mental health conditions for working age adult population. The team receives referrals mostly from GP surgeries and other sources such as Crisis team, Liaison Psychiatry and the wards. The team provides specialist support to patients with significant mental health difficulties such as mood disorders, schizophrenia, bipolar disorder, personality disorder and adult neurodevelopmental conditions such as ADHD and Autism. The Crisis Team manages access to inpatient services. The post-holder will develop effective working relationships with members of the Crisis Team and Inpatient Teams by closely liaising with these services when patients are referred to the crisis team or needing alternative to hospital beds and will be managed by the Crisis Team. Patients who need to be in hospital will be managed by the inpatient team. The post holder will join a dedicated multidisciplinary team and contribute to compassionate and evidence-based patient care and service improvement.

Main duties of the job

Clinical duties of post holder

To complete new patient assessment for patients admitted to ADHD clinic Ward, including taking a thorough history, mental state examination and complete physical examination in line with the trust policy. This also includes making an initial assessment of risks and forming an initial management plan in collaboration with the admitting nurse.

To ensure continuous monitoring of physical health of patients admitted to ADHD clinic Ward and where necessary to arrange further investigation or referral as required.

To conduct comprehensive assessment including risk assessments in the community mental health team.

To respect the rights and the dignity of patients and to consider the individual and cultural needs of patients and work with families and carers in the best interest of the patient

To conduct Medication reviews/follow up/ annual reviews for psychotropic medications and to adhere to evidence-based prescribing and monitoring of pharmacological treatment on both settings.

To adhere to guidelines of monitoring of physical health of patients on psychotropic medications on both settings.

To Prescribe safely and be involved in medication management

To participate in huddle, DDM and MDT meetings.

To be involved in Clinical leadership.

To be approved under S12 of MHA OR able to achieve within 6 months of appointment. Once section 12 approved, there is an expectation to contribute to Mental Health Act assessments
