Child and Family Practitioner - Infant Feeding Team

Halton 0-19 team are proud andexcited to be one of the 75 areas who have been awarded additional funds todevelop the Family Hub model, working collaboratively with our partners andfamilies to co-produce services to meet the needs of our population.

We are proud to be able tooffer additional training in infant mental health, parenting support and infantfeeding in our collaborate approach with our partners.

Main duties of the job

Halton is a geographicallysmall area however provides a diverse community, ranging from affluent tosocially deprived. It is an area that provides excellent experiences for HealthVisitors, especially those that wish to specialise in specific areas.

The post holder will beresponsible for assessing the needs of the local population and deliveringhealth promotion services to promote health & well-being and prevent illhealth amongst Children, Young People & Families.

You will work as part of oneof the teams in Halton, consisting of Team Leaders, Health Visitors, CommunityNurses & Nursery Nurses. This is where you will be supported whilst workingwith your own individual caseload, as well as in partnership with the newlydeveloped Family Hubs to drive forward the national agenda.

Job responsibilities

Main Duties andResponsibilities

To support delivery of theHealthy Child Programme using evidence-based practice to actively promote thehealth of children and young people.

To deliver and engage inhealth promotion programmes and public health initiatives to prevent illhealth, early identification of abnormalities and work with all clients in the0-19 service and communities to reduce health inequalities as identified withinindividual Health Plans.

To conduct health needsassessments in the home, school, clinics or community settings in a plannedprogramme of care within a variety of settings.

Offer appropriateinterventions, delivering agreed programmes of care and evaluating the outcomesfor the child, young person and family.

Establish clear and effectivecommunication links with children and young

people, parents/carers,education staff, and a range of other professionals from both voluntary andstatutory organisations.

Communicate outcomes ofassessment and future care planning to the named SCPHN health visitor or SCPHNschool nurse

Carry out home visits tochildren and families advising on all aspects of health as appropriate.

To identify and assesschildren 0-19 with complex health needs, offering an enhanced integratedservice within the multi-agency arena.

To identify, assess, reportand work independently with oversight from the SCPHN School Nurse or SCPHNHealth Visitor to safeguard and protect the health and wellbeing of childrenand young people working within safeguarding policies and guidelines. To attendChild Protection case conferences and other meetings as appropriate.

Listen appropriately and takeinto account any sensitive issues and communicate these to appropriatepersons/agencies ensuring that children are protected and that disclosures areacted upon appropriately.

To work in collaboration withother agencies to deliver services to children, young people and their families

Job Specific Requirements

* The ability to travel independently across the Trust
* The ability to work flexibly in accordance with service needs


Qualifications

* Foundation programme for assistant practitioners or equivalent e.g. NNEB,
* NVQ level 3 in Childcare
* Good standard of general education in English and Maths
* (GCSE or equivalent)


Experience

* Work collaboratively across teams, agencies, and organisational boundaries
* Experience of Working with Children/Young People/Families
* Understanding of safeguarding
* Leading and co-ordinating members of a team as appropriate
* Working within the community setting
* Experience of using spread-sheets and Databases
* Leadership Programme

* This is an internal only vacancy for staff employed at Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Teaching Hospitals NHS Foundation Trust. Please confirm that you are currently employed by one of these Trusts.


Skills & Attributes

* Competencies achieved within the workplace and attendance at a Higher Education Institute (HEI), as part of the assistant practitioner (AP) programme
* Good oral and written communication skills
* Computer literacy/competent use of electronic patient record
* Understanding of confidentiality
* Ability to organise and prioritise own workload without direct supervision
* Knowledge of Healthy Child Programme
* Knowledge of National and Local Public Health Outcomes
