To provided skilled nursing (without direct supervision) to acute/hyper acute stroke patients, in accordance with responsibilities under NMC Code of Conduct and Stroke NICE guidelines.
To work collaboratively within a multi disciplinary team (Stroke team), internal and external department/agencies, to facilitate holistic seamless care delivery for all stroke patients.
Main duties of the job
Clinical duties and manage a defined group of patients, participating in all aspects of care from initial contact/admission through discharge or transfer to rehabilitation ward. To assess, plan, implement and evaluate individual programmes of care based on an agreed model of care (Stroke NICE guidelines) and in consideration of individual need.
About us
C6 unit is an acute/hyper acute stroke unit with 32 bed capacity. Multi disciplinary team comprised of different team/agencies such as Occupational therapy, Physiotherapy, Dietitian, Speech and language therapy and clinical psychology all specialize in looking after stroke patient.
We have Pre-alert service and our Stroke Alert nurses were working closely with Ambulance (WMAS).
C6 has TIA clinic service that runs 7 days a week (including bank holiday).

* Relevant clinical post registration experience
* RN (adult nursing) with current NMC registration
* Ability to work as part of a MDT and report writing/patient documentation/ nursing assessment documentation
