and main responsibilities
1. To be first point of contact for queries from the discharge facilitators/ keyworkers
2. Work autonomously with support from the Clinical Discharge Leads, Cluster leads and wards across both UHD sites supporting with monthly meetings for information sharing and training
3. To support discharge facilitators/ keyworkers with pathway plans and signposting to appropriate resources.
4. Work within the limits of your knowledge, seeking advice from Clinical Discharge and Cluster Leads in relation to potential or actual problems in a timely fashion according to the severity of the situation.
5. Maintain accurate documentation and ensure appropriate people and services are kept up to date of changes

Qualifications
Essential criteria
6. Nursing foundation degree qualification -Nursing Associate Qualification
7. NMC registered Nursing Associate
Experience
Essential criteria
8. Experience of working in an acute hospital
9. Experience of working in a team
10. Ability to work effectively within a multi-disciplinary team under appropriate supervision
Desirable criteria
11. Evidence of involvement in support and development of less experienced staff
12. Planning complex discharge experience
13. Knowledge of related legal frameworks for discharge, social and health care
Knowledge

14. Understands the scope of the role of the Nursing Associate
15. Knowledge of complex discharge planning
16. Knowledge of when to seek advice and escalate appropriately
