Job Title: Clinical RN
Status: Full-Time (40 hrs/week)
Location: Silver Hill PACE Center, 1000 Technology Parkway, Newark, NY 14513
Department: ElderOne
Schedule: Monday-Friday, either 8:00am-4:30pm or 8:30am-5:00pm, with rotating Saturdays (non-patient care shift)
Sign-On Bonus: Up to $20,000

Your Responsibilities
* Patient Care & Service: Promote and restore health by completing the nursing process, collaborating with physicians and multidisciplinary teams, performing treatment procedures, and providing emotional support to patients and families.
* Planning & Communication: Develop and document individualized care plans tailored to each patient’s unique needs, ensuring effective communication about health status and treatment plans.
* EHR Management: Utilize Electronic Health Records proficiently, ensuring accurate documentation and communication.
* Compliance: Adhere to department protocols and regulations at all levels.
What You Bring
Required Qualifications:
* Registered Nurse license in New York State
* Valid NYS Driver’s License
Preferred Qualifications:
* 1 year of nursing experience preferred
* Prior home health, clinical, and direct patient care experience preferred

Education & Certifications
Education: AS in Nursing (Required)
Licenses/Certifications: BLS - Basic Life Support (AHA), RN - Registered Nurse (NYSED)
Physical Requirements
Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Requires frequent walking, standing, or squatting.
Pay Range
$34.32 - $45.76
The listed base pay range is a good faith representation of current potential base pay for a successful full-time applicant. It may be modified in the future and is eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
