Opportunities have arisen for Community Mental Health Nurses (Care Co-ordinator) looking to join a great team, providing specialist mental health care to the population of Bury. The Secondary Care CMHT support residents with severe and enduring mental health conditions. We aim to provide outstanding care to the people of Bury and listen to improve, in response to the needs of the population we serve.
This an exciting time for Bury CMHT. We are currently undergoing a transformation within our service as part of the National Transformations of community services and the local Living Well developments. It is an opportunity to be part of our service development, with planned investment in our staff and new role development.
The CMHT work with people aged 18 and over with severe and enduring mental health and social care needs, and provide a multidisciplinary approach to the care provided.
Care Co-ordinators are responsible for providing holistic assessment, interventions, risk assessments and care plans for people on their caseload.
To act as Care Co-ordinator, carry a caseload and work with evidence based interventions
* Assess biopsychosocial needs
* Formulate strength, person-centred care plans
* Implementation of care plans
* Assessment, formulation and monitoring of risk
* Monitor, review and evaluate care / recovery goals
* Providing education advice, emotional and psychological support
* To promote choice in service user involvement in care planning and risk management
* Liaise with all agencies, and members of the multi-disciplinary team
* Administration of depot injections either in the clients home or in the clinics if Nurse qualified.
* Supervise Community Band 5/Band 4s, students and peers as appropriate
* To maintain accurate, up to date electronic records
* To participate in the CMHT’s duty system on a Rota basis. Be able to offer a rapid response to service users’ immediate needs, including those arising from crisis.
* To work in a therapeutically informed way within the multi-disciplinary team to address the needs of service users with complex biopsychosocial needs.
* To maintain a persistent approach to engagement where there are complex mental health difficulties.
* To prioritise and work flexibly with MDT and wider agencies in order to respond to rapidly changing clinical needs.
* To promote the physical health and wellbeing.
* Ensure practice is in line with social inclusion and recovery models
* Ensure carer involvement in the service user's care.
