Are you a passionate Nurse or Occupational Therapist looking to make a real difference? Derbyshire Healthcare NHS Foundation Trust is looking for someone with experience working with older adults in the community who are living with functional mental health conditions. This brand-new role, based within the Older Adults CMHT, offers an exciting opportunity to shape meaningful support for individuals in our local area. You’ll play a key part in introducing people into the Living Well Team, helping to identify their needs and designing person-centred, compassionate support that empowers them to live their best life. Collaboration is at the heart of this role. You’ll be working alongside colleagues across the community to ensure that the right support is delivered at the right time by the right person.
About the Teams: While the role is supported by the Older Adults CMHT, you’ll also be working closely with the Living Well Team, a multi-agency group that places individuals at the centre of their care. One of the most rewarding parts of this role is being part of a team that reaches out to people who may not have had access to mental health support before. The successful candidate will need the ability to travel across the geographical area.
