Lead Complex Case Manager (Learning Disability)
Job Summary
The post holder will undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs. This includes assessment, review, and monitoring of care for those funded via NHS Continuing Healthcare within specified timeframes. The role also involves managing retrospective and appeal cases.
The post holder will act as a Case Manager for clients within Birmingham Solihull CCG receiving continuing health care funding. Responsibilities include assessing client and family needs, facilitating, planning, and evaluating service provision to ensure optimal functioning and appropriate care in suitable environments.
DUTIES AND RESPONSIBILITIES
1. Work collaboratively with health, social care, and independent sector colleagues to ensure appropriate care management processes. Present assessments to BSOL continuing care panels for ratification when required.
2. Follow the process outlined in the National Framework for NHS Continuing Healthcare, including organization, coordination, and participation in MDT eligibility recommendations verified by CCG decision-makers.
3. Manage a complex clinical caseload, including assessments, reviews (3/12 CHC reviews, unscheduled reviews, Fast Tracks), implementation of Personal Health Budgets, and ongoing case management for patients funded by CHC, Mental Health, and Learning Disabilities.
4. Ensure assessments are thorough, transparent, and multi-agency where necessary, for NHS Continuing Healthcare and NHS-funded Nursing Care.
5. Analyze supporting documentation to establish the patient's critical needs and produce specialist nursing assessments.
6. Complete new NHS Continuing Healthcare referrals within 28 days, adhering to the National Framework.
7. Undertake NHS-funded Nursing Care (FNC) determinations for service users requiring ongoing nursing care, with reviews in line with current guidelines.
8. Manage caseloads involving assessments, reviews, transitions, DOLS, LPS, SEND, and the implementation of PHBs and Care Act Assessments.
9. Support delivery and implementation of Personal Health Budgets (PHBs), DOLS, and LPS for your caseload.
10. Ensure data systems are maintained in line with policies.
11. Provide specialist advice on functional and informational matters related to the service.
12. Coordinate quality assurance activities, documentation, and reporting.
13. Apply safeguarding principles consistently and holistically.
14. Contribute to maintaining and monitoring service standards around quality.
15. Ensure mandatory training is up-to-date.
16. Manage daily inquiries to the CHC Service and address case management issues promptly.
17. Comply with NHS Codes of Conduct, Standard Business Conduct, and relevant professional standards.
18. Work towards maintaining high standards within Continuing Healthcare and collaborate with care providers to enhance patient experience.
