Overview
The post holder will play a key role in supporting the CHC department's operations by assessing a specific cohort of individuals, working in conjunction with external stakeholders, to implement an effective service, ensuring the delivery of key quality outcomes and strategic objectives. The post holder will be responsible for the delivery of patient‑centred assessments to identify any primary health care need which may result in eligibility for CHC (or associated funding stream), ensuring the national framework for NHS Continuing Healthcare and NHS‑funded Nursing Care is implemented and complied with, and where appropriate, onward referral to other agencies.
The role will ensure that the provision of care achieves individual outcomes and delivers efficient qualitative placements.
Responsibilities
* Working within a team of Continuing Healthcare Practitioners ensuring a high‑quality service is delivered.
* Managing a specific caseload of CHC eligible individuals with complex needs.
* Facilitating and contributing to multi‑disciplinary assessments and patient reviews in line with the NHS Continuing Healthcare and NHS‑funded Nursing Care National Framework.
* Ensuring the principles and duties of Liberty Protection Standards are holistically, consistently and conscientiously applied.
* Supporting the AACC team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery.
* Ensuring that all processes implemented are compliant with the NHS Continuing Healthcare and NHS‑funded Nursing Care National Framework.
* Commitment to completing all mandatory training.
* Assisting the Team Leads for Continuing Healthcare with resolution of complaints, appeals and disputes, and capturing lessons learned to mitigate these issues.
* Supporting delivery and development of standard operating procedures and processes, including those for personal health budgets (PHBs), within the team.
* Communicating information, risks, issues and dependencies, including briefings and reports.
* Maintaining professional registration and up‑to‑date knowledge of legislation, with accountability for own actions according to relevant professional code of conduct.
* Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service, and working with the team to investigate variances from target/plan and implement solutions.
* Delivering against objectives and achieving quality outcomes.
* Prioritising own workload and working to tight deadlines.
* Ensuring a person‑centred approach with individuals, families and representatives.
* Complying with NHS Codes of Conduct and Standard Business Conduct policies and other relevant professional codes where applicable.
Qualifications
Essential
* RGN or equivalent
* Significant CHC experience
* Experience of assessing individuals for health care or social care needs
* Experience of working within the NHS
* Current driving licence and access to a car

* Salary £38,682 to £46,580 a year
