Overview
A vacancy exists for Band 6 Registered Mental Health Nurses, Social Workers or Occupational Therapists to join the Salford Home Based Treatment Team at Greater Manchester Mental Health NHS Foundation Trust. The multidisciplinary team provides an alternative to psychiatric hospital admission, supporting service users in acute mental health crisis at home and supporting carers during this period. The team focuses on person-centred care, tailoring care plans to individual needs and delivering a range of psychosocial interventions. The team conducts thorough risk assessments and benefits from a supportive leadership structure for complex cases.
Responsibilities
* Provide comprehensive assessment of a service user’s mental state and social needs to aid the creation of a collaborative care plan for community management.
* Perform comprehensive risk assessment, formulation and management as part of the service user’s journey.
* Act as keyworker for a small caseload of service users, with support from the multidisciplinary team.
* Cover the duty/shift coordination role on a regular basis.
* Liaise with other agencies involved in a service user’s care.
* Lead on discharge or transfer of care for service users to appropriate agencies.
* Support and supervise junior members of staff.
About the Team and Working Arrangements
The team operates a 24/7 service; shift work, including nights, is required. Flexible working requests can be supported where possible. The post includes full supernumerary induction for the first 4 weeks, with ongoing professional development supported through appraisal and regular 1:1 supervision. There are opportunities to take part in service development projects within the role.
