Job overview

The Quality & Safety Facilitator will be responsible for the administration and monitoring of Quality and Safety related issues specifically focussing on patient safety and patient experience, whilst adhering to the Trusts quality standards. The post holder will work with Quality and Safety leads, Director of Nursing, Medical Director, fellow Quality & Safety Facilitators, Matrons, clinical and administrative colleagues.

The post holder will be an integral part of the quality and safety team and help to drive forward and improve quality within the Family and Community Care Group by working in partnership with internal and external colleagues. The Quality and Safety Facilitator will be responsible for providing practical support within the Care Group to help take forward the Quality and Safety agenda, ensuring effective systems are in place to support the management of risks, incidents and other patient safety and experience issues.


Main duties of the job

· To provide support and advice in developing and maintaining systems and processes for risk identification and management at specialty level. to be reported formally through the division and escalated to the Trusts Risk Register if appropriate.

* To work with Care Group Managers, Divisional Director of Nursing, Matrons, Clinical Quality and Safety Leads and Clinicians to triage and support the process of thorough investigations in relation to Incidents Requiring Investigation.
* Offer advice and support to colleagues’ request to undertake investigations in line with the Patient Safety and Investigation Response Framework (PSIRF).
* To be responsible for the administration and monitoring of systems for mortality reviews in the division, supporting the Quality and Safety Lead to prepare the divisional mortality and Patient Safety reports.
* To support the senior management team and Quality and Safety Lead in the administration of care group meetings in line with the Harm Free Care agenda, monitoring of action plans and sharing of lessons learnt.
* To monitor the implementation of recommendations and action plans from Patient Safety investigations.
* Support the Care Group in the triage, of and effective management of complaints, working with the Customer Relations Team and Care Group managers and Quality and safety Leads.

Detailed job description and main responsibilities

Clinical Effectiveness

* To be responsible for the administration and maintenance of systems within the division for monitoring the implementation of NICE Guidelines/Quality Standards, CAS alerts, NCEPOD, Royal College recommendations, external reviews by regulators.
* To be involved with policy development as required.
* To support the Care Group and Trust in any external/internal reviews, audits, inspections in the collation of relevant information as required supporting CQC, and other governing bodies requirements encompassing elements of patient safety, clinical effectiveness and patient experience.
* To support the clinical audit leads in the development of the annual clinical audit plan ensuring it is based on the Trust clinical audit priorities.
* Support the Care Group in maintaining a central point for data storage including reports, policies and standard operating procedures and Patient information leaflets

Patient Experience

* To assist the Care Group Quality and Safety Lead in monitoring the division’s complaints and soft intelligence to ensure adherence to timescales, identify trends, and for ensuring lessons learnt are identified and shared.
* To support the Care group by working cross-divisionally with complex issues to respond to patient and relative concerns with agreed timescales.
* To retain a central register of all complaints, concerns and soft intelligence which are formally registered within the Care Group, ensuring accurate records are maintained at all times, liaising with the central team as required.
* To ensure complaint responses are collated and delivered within timescale, and report on any delays in the process.

· To support ward managers and matrons in monitoring inpatient survey results, NHS Friends and Family Test and National Patient Surveys, PLACE inspections, identification of areas of concern and provide assistance to clinical teams when looking at action planning as a result of patient feedback.

Health & Safety

· To assist the Care Group Quality and Safety Lead in implementing and monitoring of Trust health and safety policies and procedures throughout the division.

· To support ward managers and matrons in reviewing compliance with good health and safety practice and to recommend solutions, within their competence, to improve health and safety performance

· To assist in the investigation of accidents, incidents and both public and employee liability claims.

COMMUNICATION

* To provide support and advice across the Care Group in matters relating to quality and safety and assurance. Disseminating quality and safety information as requested/necessary.
* To provide support to the Care Group and Quality and Safety Lead, in the escalation of patient safety concerns through data analysis and collation of high-quality reports for the Trust.
* To attend/participate in Care Group Quality and Safety meetings and other key meetings as requested, deputising for the Quality and Safety Lead as required.
* Contribute to, and assist with, the production of Divisional reports which may involve providing analysis of data, managing complex and sensitive information and require prolonged periods of concentration to identify specific trends.
* To highlight areas of good practice to assist in promoting Clinical Quality and Safety activities across the division, sharing learning with the wider organisation.
* To act as an effective conduit between the central Quality and Safety team and colleagues within the Care Group.

* To communicate with clinicians and managers within the directorates and be able to discern those situations where the line managers would require briefing.
* Escalate concerns to the Quality and Safety Lead and senior managers in a timely manner.

TRAINING AND DEVELOPMENT

· Supports training for Risk management, incident reporting, and complaints management within the division

· Be responsible for seeking opportunities, identifying areas for development.

· Engage in supervision and reflective practices.

ORGANISATIONAL RESPONSIBILITIES

· The post holder will be expected to work without supervision on a regular basis given the nature of the work and be expected to use their own initiative by assessing urgent/delicate situations that can be addressed by appropriate colleagues to provide solutions.

· To manage day to day administration of all quality and safety activities ensuring that quality and safety related issues are handled in accordance with Trust Policy and Procedures.

· To maintain and develop effective data storage and communication systems, maximising the use of information technology to effectively manage information using advanced keyboard skills and a range of software packages including Microsoft Word, Excel, Powerpoint, Access, Outlook and Datix.

· To assist with the formulation of Quality and Safety specific action plans, updating as necessary and reporting progress through the quality and safety groups including national, regional and local results.

· Understand the organisation structure of the Trust and have knowledge of own and other services in order to deal with inquiries from external organisations.

Significant experience of working in healthcare systems


Essential criteria

* Significant experience of working in healthcare systems
* Demonstrable knowledge and understanding of the quality and safety framework and the NHS Complaints Procedure.
* Effective oral and written communication skills in particular an ability to produce well written clear, concise reports, briefing papers and correspondence to both internal and external agencies.
* Educated to Level 3 standard GCSE or Equivalent level of English Grade C or equivalent level of skill


Desirable criteria

* Degree/Level 5 study
* Previous experience of supporting senior management teams
* European Computer Driving Licence (ECDL) or equivalent
