Mental Health Community Outreach Worker

Location: Bristol
Hours: 37.5hrs per week Mon-Fri 9-5
Salary: £12.21 per hour + £1.47 per hour holiday pay
Contract: Temp to perm

Role Overview:

This role will be responsible for working across our community outreach service supporting clients within their homes. This role will also work with clients who need enhanced crisis engagement.

Role Responsibilities:

Promote independence and provide general support
Be accountable to work across service with service users needing enhanced crisis engagement
This role focuses on our Community Outreach service; you will be required to drive in and around Bristol to meet clients in their homes.
Provide help with the upkeep of the property.
Support the implementation of service users' personal goals.
Provide emotional support.
Undertake preplacement support tasks as directed by senior staff to facilitate smooth transition into the service.
Enable service users to access and participate in their local community, including access to education and employment opportunities
Provide support to maximise service user income to include benefit claims and renewals.
Attend meetings with or on behalf of service users.
Liaise with external professionals regarding service users.
Develop and maintain an understanding of mental health sections and any associated conditions of community discharge.
Work in line with safeguarding strategies and practices.
Conduct crisis mitigation and resolution.
Undertake an advocacy role on behalf of service users ensuring that they are central to their support provision
Promote and support positive relationships with other service users, professionals, family members and the wider community
Support and work towards risk reduction and maximizing move on opportunities within a positive risk framework
Support the team to undertake support/care plans, risk assessments etc.
Take a person-centered approach with service users to develop meaningful support solutions to enable goals and ambitions to be achieved
Prompt medication, following training and assessment
Support implementation of individual budgets and finances
Develop and maintain an understanding of the mental health act
Engage in any new requirements, following training and assessment
Carry out any other duties as required to support the team Requirements:

Must have a UK driving license and access to a car
