Job Overview
This role will be based across Croydon sites, supporting the Care Transformation Programme which covers both community, and acute and urgent care. The Community Care Development Programme supports SLaM’s ambitious journey to ensure its services deliver outstanding mental health care, responding to the needs of the communities it serves – across all settings.
Responsibilities
* Support and help deliver a range of projects and programmes, multi‑tasking, producing and maintaining robust project documentation and driving delivery across clinical and service teams to enable successful change management.
* Develop community partnership and community connection projects and communications across the Trust, building an excellent understanding of the needs of the specific populations in the boroughs.
* Create and maintain connections to community groups and organisations across South London, building strong working relationships with other healthcare providers, voluntary and community groups, small businesses and community pillars.
* Seek opportunities to participate in stakeholder forums, support various workstreams, encouraging and supporting a partnership approach.
* Coordinate and drive delivery of community partnership projects within a defined portfolio.
* Liaise, develop and maintain strong relationships with healthcare and non‑healthcare organisations, including third‑sector voluntary and community organisations, faith groups, social care, housing, education and community leaders across the boroughs to enhance service delivery through partnership working.
* Reach, develop and manage partnerships from all sectors of the community.
* Work with service users, carers, staff and community members, coordinating projects and promoting co‑production service improvements and developments.
* Support the mapping of community assets, working with the Trust, local authorities and third‑sector umbrella organisations to scope existing community assets in the local landscape and create a Community Asset Database.
* Research and identify groups, organisations and populations within the local community with whom the division should develop relationships.
* Help realise identified community partnerships opportunities with a clear focus on delivery of Long Term Plan commitments for mental health.
* Utilise project management principles, supporting the Delivery Manager and other project leads, including wider clinical/operational staff and partners such as local authorities, service users and carers, and third‑sector organisations.
