We are seeking a proactive and compassionate Community Health and Wellbeing Coach to join our team. The role involves working directly with local residents to improve health, wellbeing, and confidence through one-to-one support, group workshops, and community outreach.
Salary: £22,588.50-£27,000.00 per year

Role Description
Using a holistic, strengths-based approach, you will help people overcome barriers such as poor health, low confidence, unemployment, digital exclusion, and social isolation.
Key Responsibilities
* Provide tailored advice, guidance, and practical interventions to help people improve their physical, mental, social, and economic wellbeing.
* Support individuals to access health services, training, employment support, and digital/health literacy resources.
* Use a problem-solving and strengths-based approach to build confidence and enable positive lifestyle changes.
* Maintain a caseload, ensuring timely and effective support for each individual.
* Plan and deliver group workshops and community sessions on topics such as health, wellbeing, digital literacy, and employability.
* Carry out community outreach to engage individuals who are at risk of poor health or social exclusion.
* Use social media and other communication channels to promote activities and engage the wider community.
Partnership & Networking
* Work closely with GP practices, health services, schools, employers, and community/voluntary groups.
* Develop strong referral pathways and connect individuals to appropriate services and opportunities.
* Represent Colebridge Trust at community events and partnership meetings.
Monitoring, Reporting & Funders
* Keep accurate and confidential records of interventions and outcomes.
* Produce monthly monitoring reports in line with funder and organisational requirements.
* Provide case studies and feedback to demonstrate impact and support programme development.
Person Specification
* Experience of working in community health, outreach, or intervention-based roles.
* Ability to provide advice, guidance, and support across areas such as health, wellbeing, employability, and digital skills.
* Strong communication and interpersonal skills, able to work with individuals from diverse backgrounds.
* Experience of facilitating group workshops or community sessions.
* Confident in using social media and digital platforms for community engagement and promotion.
* Strong organisational skills, able to manage a caseload and meet reporting requirements.
* Full driving licence and access to own car.
* Understanding of safeguarding, confidentiality, and professional boundaries.
* Knowledge of the North Solihull area and its communities.
* Experience of partnership working across health, social care, or voluntary/community sectors.
* Relevant qualifications in community development, health improvement, or advice and guidance.
