Key Responsibilities
Carry out comprehensive assessments for individuals presenting with non-opiate substance misuse
Develop and review personalised recovery care plans with service users
Deliver structured one-to-one and group interventions, including motivational interviewing, relapse prevention, and CBT approaches
Provide harm reduction advice to minimise risks associated with substance misuse
Support clients in accessing wider services such as housing, healthcare, education, and employment
Manage a diverse caseload, ensuring regular reviews and updates to care plans
Identify and escalate safeguarding concerns appropriately
Collaborate with families, carers, and partner agencies to deliver holistic support
Maintain accurate case records in line with confidentiality and data protection policies
Gather and report outcome data to contribute to service monitoring and improvement
