Hours 35 Salary 26,992.35 Contract Permanent (subject to continued funding) Reports to W3RT Line Manager Main purpose of the role: The GP Link Worker supports patients to improve their health and wellbeing by addressing non-medical needs. They work with individuals referred by GPs and other primary care professionals to connect them with community services, voluntary organisations, and local support that help tackle social, emotional, and practical issues affecting health to ensure problems and issues do not worsen and lead to ‘crisis’. Responsibilities: Receive referrals from GPs, nurses, and other primary care staff Meet patients (face-to-face, phone, or online) to understand their needs, goals, and circumstances Co-produce personalised wellbeing plans with patients To create short, medium and longer-term goals with clients, through on-going personalised care and support planning, to provide appropriate support and achieve positive outcomes. To undertake training as required and be prepared to travel throughout appropriate areas to attend any relevant meetings. Ensure that social prescribing referral codes are inputted to EMIS/SystmOne/Vision and that the person’s use of the NHS can be tracked, adhering to data protection legislation and data sharing agreements with the Integrated Care Board (ICB). Comply with Health & Safety policies including, but not limited to, those on lone working, manual handling and infection control. Ensure service users’ health and wellbeing is preserved and safeguarding policies and procedures are followed at all times. Work with colleagues to ensure that health and safety legislation and risk assessments are understood and implemented. Work in partnership with Primary Care Networks, the District and Borough Councils, Housing Associations, local Voluntary Organisations and Forums to ensure that the project is effectively used and understood. Identify gaps in services and relay to management. Adhere to policies and procedures regarding data protection and confidentiality.
