Hours: 21 hours per week, 9:00 - 16:30 Monday, Tuesday, Thursday This is a 9 month fixed term contract. Rate of pay: £22.68 per hour Key Results Summary responsibilities Occupational Therapy assessments of clients in our 12 P2 beds Setting and reviewing rehab goals and rehab programmes, determining accurate discharge dates and post-discharge equipment and care requirements, and ordering equipment from the local NHS community equipment stores (Medequip) Attending MDT meetings Home visits Working in a team with other OTs, Physios and support staff. OT assessments of permanent and short-term residents of the care home and of residents in the independent living flats with the aim of reducing the risk of falls, rehabilitation, posture management and promoting movement, independence, and social engagement. Therapy treatments including education, functional exercise (e.g. mobility and transfers practice) and advising on equipment. Residents-centred multidisciplinary team working. Helping residents to manage acute and long-term conditions. Safely and appropriately delegate therapy tasks to therapy assistants Maintain accurate records necessary for the role. Communicate readily and effectively with residents, their families, colleagues, and other individuals from outside the organisation. Skills, experience and qualifications Essential BSc (Hons) Occupational Therapy Member of RCOT and registered with HCPC. Good IT skills. Experience in working with older people – including frailty and dementia care. 2 years post-graduate experience Experience working with therapy assistants Experience working in NHS Up-to-date professional knowledge Evidence of personal development Excellent team worker Motivated and enthusiastic worker Orthopaedics experience Community/home visit experience Enthusiasm for improving the wellbeing of older people Approachable Experience in hospital discharge planning and rehabilitation Desirable Experience of working within a care home Experience working in BNSSG Experience ordering equipment using TCES Community Experience in wheelchairs and/or seating Accredited referrer for BWCS Neuro experience Driving license and a car Other Skills and experience Communication Interprets and explains information to others. Provides and receives some complex, sensitive or contentious information. Persuasive, motivational, empathetic, reassuring skills. Confident negotiator. Shares knowledge and trains others on the job. Communicates openly and often with immediate and related teams. Objective review of complex matters. Thinking Style Analyses complex facts or situations. Plans and organises a range of complex and interlinked activities and tasks (project planning). Uses sophisticated technical and problem-solving skills to enhance service area/discipline. Regular report writing, data analysis and recommending actions. Develops and maintains policies and procedures in own discipline. Aware of longer-term impact of decisions. Well-being and Values Demonstrate sensitivity to individuals’ needs. Strong emphasis on colleague engagement and well-being. Successfully embeds strategic/values-based goals of service area. Considers impact of initiatives on well-being and engagement of stakeholders.
