Why this role would be great for you
As a Community Housing Support Worker at YMCA Together, you will be an empathetic and caring individual committed to providing unconditional support for those experiencing mental health issues. Your role involves creating a safe environment that empowers residents to make positive changes in their lives.
This service supports vulnerable young people (ages 18-25) who are experiencing or at risk of homelessness or rough sleeping with medium to high support needs. You will facilitate recovery, self-development, and prepare them for independent living, ensuring they are supported and empowered to make positive life choices and develop tools to live free from homelessness.
As a support worker, you should have experience working with young people and engage them using familiar tools, language, and platforms such as technology and social media. You will recognize that many individuals have experienced trauma and will work with an understanding of this. You will collaborate with our Assistant Psychologist as needed and manage a caseload of five individuals, focusing on intensive one-to-one support to help them meet their goals.
What your week may look like
1. Welcome new service users, inducting them into their new properties and identifying immediate support needs.
2. Ensure service users understand their rights and responsibilities.
3. Support service users in achieving their goals as outlined in their support plans and empower them to reach these aims.
4. Manage, update, and communicate risk assessments in accordance with YMCA policy.
5. Ensure support is shaped by and understood by service users.
6. Hold planned key-work sessions and ad hoc support using a person-centred reflective approach.
7. Assess and plan support around individual needs, regularly reviewing and developing support plans with service users and relevant professionals.
8. Maintain up-to-date records on support planning systems (MainStay and DAVE).
9. Conduct regular risk assessments and keep all relevant parties informed.
10. Assist service users with future tenancy planning, ensuring they acquire skills for independent living.
11. Motivate and challenge service users to achieve their goals.
12. Coordinate with external agencies for specialized support when appropriate.
13. Encourage participation in service planning and decision-making through meetings and forums.
14. Address complaints and safeguarding concerns promptly.
15. Demonstrate creativity and commitment in supporting vulnerable individuals.
This role may evolve based on the changing needs of the organization and the environment.
