This position offers a hybrid working arrangement and an opportunity to make a meaningful impact by coordinating multi-agency support and empowering individuals and families to achieve positive, sustainable outcomes.

About the Role As a SEND Key Worker, you will coordinate and facilitate the assessment of need and the development of Education, Health and Care Plans (EHCPs) in line with the SEND Code of Practice and relevant legislation. You will act as a central point of contact for individuals with SEND and their families, ensuring that assessments, support planning and interventions are timely, holistic and reflective of the individual’s strengths, needs and aspirations.

You will also collaborate closely with Social Care, Educational Psychology, Health professionals and external agencies to ensure a consistent and joined-up approach to supporting individuals with SEND across Staffordshire.
