Responsibilities
* Deliver immediate and continuing nursing care to patients within their own homes.
* Prioritise, assess, plan, evaluate and deliver all relevant aspects of nursing care to an agreed and expected high standard.
* Work as a key member of the team and contribute to processes that promote patient safety and quality of care in line with clinical governance frameworks.
* Gain competence in clinical skills to enable delivery of quality patient care within the service.
* Take an active role in contributing to developing nursing practice.
* Maintain effective communication internally and externally to the organisation and take responsibility for assessing, planning, implementing and evaluating evidence-based care for patients.
* Provide palliative care to patients and their families in their own homes.
* Provide ongoing nursing care and support to patients and carers, promoting the role of the nurse and clinical excellence.
* Identify patients whose conditions become unstable and liaise with doctors and acute services appropriately.
* Be responsible for documentation in line with current practice and policies.
* Ensure that patients can make an informed choice about their current and future care needs, ensuring care is patient centred and confidentiality is maintained in line with Trust policy.
* Prioritise own workload and work autonomously when necessary.
* Provide support, guidance, and supervision to junior staff.
* Assess the level of ongoing nursing needs for people who may need long term care.
* Liaise with the patient's GP to provide sufficient information for ongoing primary care to be provided safely and efficiently.
