Job Overview This is an exciting opportunity to progress the Health Inequalities Agenda within the Trust, delivering a digital inclusion service and implementing interventions that address inequalities in access, care and outcomes, creating sustainable change for patients. The purpose of this role is to support patients who are likely to experience blocks to how they access or experience care and the aim is to provide equitable access to care for all our patients and advancing the NHS Digital Inclusion strategy. Main duties of the job You will establish links with community service providers and gain up to date knowledge of the services they provide to reduce digital exclusion. You will support patients who are digitally excluded to access training and resources and help them develop the skills and confidence to access virtual care and electronic systems. You will be proactive in identifying opportunities to reduce digital exclusion, improve the uptake of digital care and self-management of health and wellbeing. You'll work in close collaboration with colleagues across the Trust, in the community and local authority partners. This is primarily a patient-facing role and the you will have direct contact with inpatients (people staying in hospital) and outpatients (patients accessing treatment at appointments).
