Overview
Opening at Norwich Community Hospital in 2025, the new Willow Therapy Unit provides a supportive and comfortable environment for 48 patients. The restorative environment at Willow Therapy Unit enables patients to make steady, step-by-step improvements and gain independence as they prepare to return to the community. Patients are supported to play an active role in their own care and feel confident about performing daily activities when they leave.
Care is delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and wellbeing. NCHC is seeking an experienced and enthusiastic Occupational Therapist to join our team in providing reablement care to a range of patients across a 7 day service including Bank Holidays. Shifts are 11.5 hours per day (7am-7:30pm). Applications for full or part time hours are welcomed.
Responsibilities
* Be responsible for delivery of care in an reablement setting.
* To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
* To support patients being as independent as possible within their functional limits.
* To work as part of a multidisciplinary team within a quality framework to ensure the highest standards of care are met.
* To support and deliver innovation within discharge practice and pathways.
* To work with system partners including the voluntary sector to ensure that patients\' experience is a seamless transfer of care both into and onward from the unit.
* Be responsible for the delivery and co-ordination of care to a named group of patients.
* Work proactively within the team to achieve unit and trust goals and objectives.
* Work with and promote recognised policies, protocols, and guidelines.
Additional duties
* To contribute to the delivery of therapy assessments and treatment plans for patients with long-term conditions, palliative care and rehabilitation needs to achieve quality of life and independence where possible.
* Work within the integrated team to facilitate early discharge from hospital.
* Work within the integrated team to prevent unnecessary admission to hospital.
* Work with all healthcare professionals, and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
