Are you looking for a new and exciting community-based role, working within a supportive and friendly team?

This band 6 post will be for 22.5 hours per week on a permanent contract. The service cover for the C ommunity C hildren’s N ursing (CCN) team is 8am-8pm Mon-Friday, including Bank Holidays.

This exciting and varied role is looking for someone with post registration experience in paediatrics, with experience in teaching and assessing. Experience with working with children with complex needs and safeguarding experience is desirable. The CCN team strive continuously to improve the lives of those children and families for whom they care, acting as not only clinicians but advocates and advisors too.

You will be based primarily at L11, University of Reading, London Rd, RG1 5EX but will also cover the areas of Reading, West Berkshire and Wokingham.


Main duties of the job

Holding a caseload for children with a wide range of medical and nursing needs across Reading, West Berkshire and Wokingham

Working within the CCN team whilst improving cohesive working between the CCN and specialist school nursing team.

Acting as a keyworker for children and families and being the link between hospital and the community. This will include signposting to other appropriate sources of support and liaising with other professionals involved as applicable.

Providing excellent standards of clinical nursing care, support and advice to children and their families, including regular reviews, visits and attending clinic appointments as needed.

Being part of multidisciplinary meetings with allied professionals, including discharge planning meetings, safeguarding meetings, team meetings etc.

Central line maintenance and all associated cares, including taking bloods as required .

Administering medications such as IM injections and intravenous antibiotics.

Raising concerns appropriately and being involved with safeguarding processes and attending any Child in Need / Child Protection meetings as required .

To be part of the extension of the 8am-8pm service in conjunction with the other senior nursing staff as required .

To be part of the Rapid Response rota alongside the Police if a child dies unexpectedly in the community

Previous Experience

* Proven experience in working with children and young people who have complex health needs and an associated learning disability
* Safeguarding knowledge and experience
* Training Clinical skills
* Children’s Community Nursing Experience


Knowledge, Skills & Abilities

* Excellent Communication Skills: Ability to demonstrate high level communication skills dealing with complex situations that arise within the community setting and outside agencies
* Good communication skills incorporating communication with children\young people and their carers
* Effective time management skills and prioritise workload
* Approachable, confident, empathetic, trustworthy, passionate about delivery of high quality nursing care
* To be able to organise and manage own work and caseload
* Ability to use own initiative and work independently
* Good understanding of IT skills
* Knowledge of palliative and end of life care
* Wide range and nursing skills, knowledge and experience, including care of tracheostomies, enteral feeding devices and epilepsy
* Ability to assist and motivate others
* Able to manage junior staff
