Job summary
Blackwater Medical Centre has a fantastic opportunity for a nurse prescriber/advanced nurse practitioner to join our team on a full-time basis, but we will consider part-time hours for the right candidate.
You will be managing clinical cases and dealing with presenting patient needs within a primary care setting, while participating in nurse-led minor illness clinics.
Main duties of the job
1. Providing the choice of direct access to an NP, in the practice, to the community team & over the telephone, for the general practice population.
2. Making professionally autonomous decisions.
3. Providing a first point of contact for patients.
4. Instigating necessary invasive & non-invasive diagnostic tests or investigations & interpret findings/reports.
5. Prescribing safe, effective & appropriate medication as defined by current legislative framework.
6. Providing safe, evidence-based, cost-effective, individualised patient care
7. Offering a holistic service to patients & their families.
8. Promoting health & well-being through the use of health promotion, health education, screening & therapeutic communication skills.
9. Referring patients directly to other services/agencies.
10. Working as part of the practice team in developing & delivering specialist primary care services.
11. Screening patients for disease risk factors & early signs of illness.
12. Working with colleagues to ensure that National Service Frameworks are being delivered.
13. Working with multi-disciplinary team to promote integrated & seamless pathways of care.
14. Contributing to the practice achieving its quality targets.
15. Participating in the identification of community health needs & develop strategies to address them.
16. Helping develop & set up new patient services & participate in initiatives to improve existing patient services.
17. Being aware of & contributing to the planning & delivery of practice-based commissioning.
