Complex Emotional Needs Care Coordinator / Key Worker

An exciting opportunity has presented itself for a motivated and enthusiastic Key Worker to become a full-time member of the new Complex Emotional Needs Service.

This role is for a passionate and energetic professional to join a dedicated team working closely together to provide high-quality, evidence-based treatments in a psychologically informed way. This role is ideal for someone with a specialist interest in working with individuals with complex emotional needs and developing their therapeutic knowledge and skills.

The whole team values clinical supervision and reflective spaces, utilizing skills and psychological models to expand their thinking.


Main duties of the job

* Work as a core member of the community Complex Emotional Needs Service.
* Collaborate with senior clinicians to ensure the treatment programme is delivered, monitored, and evaluated, including group work as required.
* Commit to effective team working and coherent delivery of treatment and therapy.
* Promote a high standard of patient care.
* Work within the Trust's policies and standards set out in the Community Mental Health Teams Operational Policy and CEN Policy.
* Participate as a member of the Multi-Disciplinary Team in Community Mental Health Services, transitioning to the Complex Emotional Needs Service upon implementation.
* Deliver all aspects of Care Coordination to the highest standards with available resources, including supervision and consultation with Key Workers and staff.
* Provide in-reach support to the wider Navigo teams and Primary Care Networks.

Job responsibilities

* Provide specialist assessments using complex data, psychological tests, self-report measures, rating scales, observations, and interviews.
* Formulate and implement plans for psychological treatment or management based on evidence-based methods.
* Evaluate and decide on treatment options considering individual historical and developmental factors.
* Manage a caseload including individual and group therapy.
* Provide clinical advice, guidance, and consultation to other professionals for client formulation, diagnosis, and treatment planning.


Person Specification


Additional Requirements

* Driving Licence - Must hold a full driving licence and have access to a car for work purposes.


Knowledge

* Assessment, planning, implementation, and evaluation of care plans.
* NICE guidelines, NSF, Clinical Governance.
* Current research in the field.
* Person-Centred Care.
* Audit processes.


Qualifications

* 2-3 years post-professional qualification.
* Relevant training and courses.


Experience

* Working with Personality Disorders/Complex Emotional Needs.
* Working with adults of all ages.
* Within a CPA framework.
* Risk Management training and experience.
* Liaising with other teams/services.
* Innovative practice in nursing/social work.
* Key-worker/care co-ordinator experience.
* Recruitment and selection involvement.
* Care management experience.
