Stockport NHS Foundation Trust Community Neuro Rehabilitation Service (CNRS) is seeking an enthusiastic, innovative, and experienced Assistant Practitioner to join our friendly, forward-thinking team. The role involves providing rehabilitation for a diverse range of individuals with stroke and neurological conditions within Stockport.


Main duties of the job

Based in the community, you will work within a comprehensive multidisciplinary team including Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Therapy Assistants, and Admin staff. You will be a valued team member, sharing knowledge, skills, and learning opportunities.

Your responsibilities include facilitating early supported discharge of stroke patients from the stroke unit, and working with patients with acute and long-term neurological conditions to provide rehabilitation in their homes and other community settings, thereby improving their quality of life.

Job responsibilities

You will work closely with experienced clinicians and support Band 3 Therapy Assistants. Our team is supportive, caring, and proactive, dedicated to providing holistic, person-centered care for individuals with stroke and neurological conditions in the community.

The Trust is committed to safeguarding and expects all staff to promote welfare and comply with policies. An enhanced DBS check (£54.90) and annual update service subscription (£16) are required.


Person Specification


Education & Qualifications

* Foundation Degree in Health and Social Care or equivalent
* GCSEs in Maths and English (Grade C or above) or equivalent
* Evidence of Continuing Professional Development
* ECDL or equivalent IT qualification


Knowledge

* Understanding of physical disability and relevant legislation
* Knowledge of therapy interventions and when to seek advice
* Awareness of cultural diversity, dignity, respect, and communication barriers
* Knowledge of health policies


Experience

* Experience as a therapy assistant or Assistant Practitioner in hospital or community settings
* Experience working with diverse patient populations and in multidisciplinary teams
* Experience in staff induction and education


Skills & Abilities

* Excellent communication and organizational skills
* Positive interpersonal skills and professional conduct
* Ability to work independently and reflectively
* Flexible, adaptable, and observant
* Understanding of health, safety, and risk management
* Ability to handle sensitive situations and teach practical skills

* Willingness to travel; driver’s license required


Disclosure and Barring Service Check

This role requires an enhanced DBS check (£54.90), with costs deducted from your first salary, and annual subscription (£16) for the DBS update service.

Salary range: £27,485 to £30,162 per annum, pro-rata.
