The post holder is to standardise the management of prescriptions bybecoming a single point of contact for medication and prescription requests andqueries. This will improve medicines safety by preventing errors, reducingwaste and containing costs. The post holder will work as part of amulti-disciplinary team signposting requests and responses to pharmacists,receptionists, GPs and ANPs, to improve patient experiences and health outcomesalso improving satisfaction for staff by reducing workload.

Main duties of the job
Action system online, and NHS app prescription requests sentonline directly from patients within 1 working day after 6pm. Actionhandwritten paper prescription requests forwarded on to us via reception within2 working day after 6pm.
Actionmedication requests and queries from pharmacies sent via the practiceindividual NHS mails within 1 working day after 6pm. Action external medicationrequests and queries from care home sent via the practice individual NHS mailswithin 1 working day after 6pm. Action paper, postal requests from appliancecontractors. Deciding which types of review are needed (of the 19+) for apatient and when they are required. Send appropriate booking links for reviewwhen there is a need for an appointment. Knowing the different elementsrequired for disease reviews e.g., which blood tests, whether BP and urine arenecessary. Signposting any acute requests to the appropriate group ofclinicians including relevant information required to issue or decline therequest if appropriate. Electronic repeat dispensing repeat medications forpatients that meet a specific criterion to reduce medication requests frommonthly/bi-monthly to yearly/bi-yearly. Action early requests for reasons suchas holidays etc. knowing where more information is required. Issuing clinicianonly issue set medications for patients that are only allowed to have theirmedications on the same day.

Job responsibilities
Recognising when a patient may be overusing a medication and mayrequire monitoring. Issue 12 weekly dosette prescriptions to pharmacies for allAffinity sites when due so the patient nor pharmacy need to order these. Actionany medication changes required for dosette boxes, informing all necessaryparties (the patient/pharmacy) within a timely manner, issuing any prescriptionrequired to fulfil the required boxes. Issue 3 monthly regular prescriptions topharmacies for care home residents so the staff nor pharmacy need to orderthese. Cleaning up repeat medication lists making sure issue durations arecorrect, removing any medications not used and requesting medications issued byother places are put into other medication etc. Liaise professionally andpleasantly with patients regarding any medication queries they may have.Learning clinical knowledge e.g. knowing what medications are for whatillnesses/diseases, what hormones HRT is composed of and subsequently what isneeded based on reproductive organs Knowing the different elements/parts todiabetic materials and machines to understand better what the patient needs.Knowing the different elements/parts to appliance materials and catheters tounderstand better what the patient needs. Knowing alternative names of the samemedication.
Person Specification
Experience
* Demonstrable experience of working in an administrative environment and working with GP clinical systems
* Experience working in a health care environments, particularly one dealing with medicines
* Knowledge of NHS issues
* Understanding of Data Protection legislation
* Familiarity with the names of common medication
* Working knowledge of Systmone or equivalent medical system
Skills and knowledge
* Clear communicator with good writing, date entry and telephone skills
* Ability to work effectively as part of a team
* Able to work on own initiative and prioritise workload
Qualifications
* GCSE English Language, Grade C or above (or equivalent)
