The Community Mental Health Nurse is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change, and promote health, best practice, patient safety, and experience among our caseload of older adults.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
1. Carry out and coordinate comprehensive, systematic nursing assessments considering mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with the service user and others through interaction, observation, and measurement.
2. Work collaboratively with other disciplines, service users, and their carers to develop a holistic, person-centred care plan based on assessment findings.
3. Deliver and evaluate safe, person-centred care in partnership with the service user and their carers to support recovery.
4. Act as a clinical leader supporting best practice in line with NICE guidance.
5. Facilitate and review approved Quality Improvement initiatives within the care setting.
