The Community Rehabilitation Team is a new development and continues to evolve and expand. You will play a pivotal role in establishing the team's culture, processes, and impact, providing a clear pathway for your career progression into management.
You will lead a dedicated multi‑disciplinary team of occupational therapists, psychologists, and support workers. Together, you will work in partnership with community mental health teams to deliver vital rehabilitation support and interventions, empowering individuals on their recovery journey.

Main duties of the job
The post holder will provide leadership, managerial and supervisory responsibility to a group of practitioners working as a multidisciplinary team; providing assessments and interventions within adult community mental health services.
The number of staff supervised will determine how the workload is managed between active clinical work and management responsibilities. The post holder will assist the clinical manager and work alongside the other team managers in continually developing the service and ensuring the delivery of an effective Community Rehabilitation offer.
Partnerships will include GPs, other mental health services, learning disability services, social care, physical health services, consultation to professionals in the primary care services and other adult mental health services including the voluntary sector.
Priority areas for this post will be agreed annually via the appraisal process.

Person Specification
Qualifications
* Professionally recognised nurse, Social Services or AHP qualification.
* Previous experience of working in a community mental health setting.
* Evidence of further formal management study and/or qualification.
* Training in a modality of psychological therapy, family interventions and/or PSI.
Experience
* Minimum of 3 years experience as a practising professional in an acute mental health environment.
* Knowledge of the legislation surrounding mental health and vulnerable adults.
* Able to integrate and work within a multi-disciplinary team and in complex service structures.
* Able to work flexibly and deliver a service through working practices that are dictated by service user/carer need.
* Experience in both health and social care.
* At least 1 year’s management experience in a community mental health team or service.
