Responsibilities

As a senior member of our team, you will be a clinical leader, an expert practitioner, and a vital support to your colleagues. You will manage complex caseloads, drive service improvements, and mentor junior staff in a supportive environment.


Clinical Practice & Caseload Management

* To manage a defined, complex caseload, using advanced clinical assessment and diagnostic skills to provide high-quality, evidence-based care to patients within the AACHC service.
* Prescribe, review, and evaluate the effectiveness of medication and treatment therapies in line with patient group directions and professional standards.
* Lead and deliver a range of clinical interventions and procedures, acting as an expert resource for the multi-disciplinary team (MDT).
* Ensure the provision of comprehensive health education and promotion to patients and their families.


Leadership, Supervision & Management

* Provide day-to-day clinical leadership, professional supervision, and mentorship to junior nursing staff, students, and support workers.
* Deputise for the Band 7 Team Lead in their absence, ensuring the smooth and effective running of the service.
* Contribute to the induction, training, and development of team members, fostering a positive learning environment.
* Participate in the planning, development, and evaluation of clinical services, proposing and implementing changes to practice.


Quality, Safety & Governance

* Act as a champion for clinical governance, leading on risk assessment, incident reporting, and the implementation of resultant action plans.
* Undertake clinical audit and evaluate standards of care, using the findings to improve service quality and patient outcomes.
* Ensure that care is delivered in accordance with all local and national policies, procedures, and guidelines, including the NMC Code.
* Promote a safety culture and ensure all practice is in line with safeguarding principles for both adults and children.


Communication & Teamwork

* Work autonomously whilst functioning effectively as part of an MDT, building strong, collaborative relationships with colleagues in health, social care, and voluntary sectors.
* Communicate complex and sensitive information effectively with patients, carers, and team members, ensuring a seamless patient journey.
* Maintain accurate, timely, and comprehensive patient records in line with professional and trust standards.


Professional Development

* Take responsibility for your own continuous professional development, maintaining your NMC registration and fulfilling revalidation requirements.
* Keep up-to-date with professional practice and new developments, incorporating evidence-based findings into your work.


Person Specification


Essential Qualifications

* Registered Nurse (Adult/Child/Mental Health/Learning Disability) with current NMC registration.
* Post-registration qualification or significant equivalent experience relevant to the AACHC setting.
* Evidence of continuous professional development.


Essential Experience & Skills

* Substantial post-registration experience, including within a relevant acute, community, or children's setting.
* Proven experience in managing a complex clinical caseload autonomously.
* Demonstrable experience in mentoring, supervising, and supporting junior staff.
* Excellent clinical assessment, care planning, and evaluation skills.
* Strong leadership qualities and the ability to motivate a team.
* Outstanding communication, interpersonal and organisational skills.
* Competent in using IT systems for patient records and data management.


Desirable

* Non-Medical Prescriber qualification.
* Physical assessment skills course or equivalent.
* Experience in clinical audit and service improvement projects.
