This is a unique opportunity to play a pivotal role in shaping integrated care pathways as we blend operations with our START (Short Term Assessment & Reablement Team) colleagues—supporting earlier discharge and home-based assessment. This post is one of several new opportunities that have arisen from additional funding from ScotGov for Moray Health & Social Care partnership to enhance its delivery of the NHS Grampian Home First and Unscheduled Care Improvement programmes. Leadership & Influence : As a senior clinician, you’ll provide supervision to team members, contribute to service development, and influence strategic direction under the guidance of the Lead Occupational Therapist. Collaborative Innovation : Work within a therapy-led, multidisciplinary team including OT, PT, Nurse Practitioners, Therapy Support Workers, and Social Care Assistants—delivering goal-oriented, person-centred rehabilitation in patients’ homes. Quality & Improvement : Be part of a service that values evidence-based practice, quality improvement methodology, and continuous evaluation to enhance patient outcomes.
