Overview
In addition to being an expert in your field in possession of extended competencies, you will need to demonstrate excellent communication skills to illicit an accurate, holistic assessment of patient needs as part of a case management approach. We offer training and support to move towards greater engagement with our patient population.
Responsibilities
* Work within a dedicated referral centre and use critical thinking and negotiating skills to aid the triage process.
* Manage a defined caseload of patients with delegated responsibilities to your Band 5 nursing team to support continuity of care for patients as part of the nursing model of care.
* Collaborate with the services and other professions within the Gloucestershire Health and Care NHS Foundation Trust, including ICT (Integrated Community Team), Therapy colleagues, GP surgeries, Rapid Response and Complex Care at Home to ensure the right care is delivered at the right time by the right professional for patients.
* Prior experience with electronic patient record IT systems (e.g. System One) is advantageous; full support and training will be provided.
