Salary available on application. 6 hours per week, Wednesdays based at Llanfair Health Centre; may occasionally require attendance at Dwyran surgery. This is a temporary contract initially planned for Winter pressures.
Job Description
Job Title: Advanced Nurse Practitioner in General Practice. Reports to GP Lead and Practice Manager. Clinically accountable to GP partners.
* Provide care for patients presenting with undifferentiated problems from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
* Perform face-to-face assessments and decision-making within professional boundaries, collaborating with the general practice team to deliver policy, procedures and nurse leadership as required.
* Work within NMC requirements for advanced practice.
Key Responsibilities
* Clinical practice: assess, diagnose, plan, implement and evaluate treatment for undifferentiated or complex needs; manage both acute and chronic conditions; prescribe and review medications within scope; promote adherence and provide information on medications.
* Communication: use sensitive and adaptable communication to inform and obtain consent; support patients and carers, considering cultural and individual needs.
* Delivering a quality service: maintain records, comply with NICE guidelines, participate in governance, audits, and policy implementation; promote learning across the practice and organisation.
* Leadership and team development: foster a learning environment, support staff development, and engage stakeholders in service improvements.
* Other: maintain knowledge of safeguarding responsibilities and local guidance; participate in vaccination programmes and immunisations; provide care in line with local and national strategies.
Person Specification
Experience - Essential: Advanced clinical practice skills and clinical examination skills. Desirable: Management of patients with chronic conditions and complex needs with appropriate referral.
Qualifications - Essential: MSc level Advanced Nurse Practice; Nursing/health degree; Independent prescriber qualification. Desirable: (same as essential).
